Not sure how to do a full mask template but here is a tutorial that Autisticwonder wrote on how to make a mask shape. Maybe you already know how to do that, but maybe this will help. It was wonderful for me!

Hi MaggieM, I'm going to give you a link to a thread that has tons of mask's in them... (it's Shelby's request for Help), there I explained how to make the masks.. I hope it helps..OOOH, and it you look through all of WONDERFUL WONDERs Mask template threads you will find the "grey" papers and tons of instructions..

Masks

Thank you so very much for this help and the suggestions. I love the masks and I love scrapping, but I would like to start creating some things of my own, in addition to all the learning.
